Medication Therapy Management

When you are taking more than one medication or supplement, managing your health can be more difficult. Through the HealthEast Medication Therapy Management program, our specially-trained pharmacists work closely with you and your health care providers to ensure your medications are the best for your health conditions, lifestyle and budget.

You may benefit from an appointment with a pharmacist if you:

  • Take more than one medication or are being treated for more than one chronic health condition
  • Would like to know how your medications work and why you are taking them
  • Are concerned about the potential for drug interactions – even with over-the-counter medications and supplements

How Medication Therapy Management can help

Medication Therapy Management can help you take control of your health by learning more about the medications you are taking. You'll meet with a pharmacist at the clinic who will help:

  • Address your questions and concerns as they relate to your medications
  • Create individualized goals and personalized interventions to meet your needs
  • Help you understand the role of medications as it relates to your health care

What to expect

You'll be asked to bring all prescriptions, vitamins and over-the-counter medications to your appointment. During the visit, the pharmacist will evaluate your medications and address any questions or concerns you have. Any changes or recommendations that are made to your medications will be done in collaboration with your primary care provider.

Follow-up visits with your pharmacist will be scheduled as needed to ensure any changes made to your medications during the previous visit are working well for you.
