Community Health Needs Assessment
Responding to the health needs of the community, especially the most vulnerable among us, is core to achieving our HealthEast vision of optimal health and well-being. To do so, we must have an understanding of these needs and develop a deliberate approach to addressing the needs. An assessment is conducted at least every three years to help determine which programs and activities contribute to the health and vitality of our neighbors.
HealthEast conducted Community Health Needs Assessments (CHNA) in 2017, 2015 and 2012.
HealthEast’s Community Health Implementation Plan describes our plan for improving community health, and how we will measure our impact.
Community health improvement initiatives
The central goal of community health improvement is to produce measurable improvements in health status through strategic, evidenced-based programs. We will do this by partnering with community organizations.
