Physician approval
Your participation in the program may require physician approval.
Determine if you need physician approval by answering the seven questions at the top of page 6 of the Registration Packet. If physician approval is needed, take the physician medical release form (page 6) to your clinic to complete and fax to Ways to Wellness at 651-326-8729.
