Then, fill out a Registration Packet. Please bring your completed packet to your first appointment.

Physician approval

Your participation in the program may require physician approval.

Determine if you need physician approval by answering the seven questions at the top of page 6 of the Registration Packet. If physician approval is needed, take the Physician Medical Release Form (page 6) to your clinic to complete and fax to Ways to Wellness at 651-326-8729.

The Oak Center building is located on the Woodwinds Health Campus ACROSS the parking lot from the main hospital entrance.
