Thank you for your interest in joining the Healtheast Telestroke Program. If you want to establish a Telestroke program at your hospital, we can help you get started.

Hospital application

Complete the hospital application and submit it via fax (651-326-9118) or scan and e-mail it to Amy Castle. This document gives us your key contacts, along with important information about your hospital and staff.

After we receive your application, our implementation team will work with your team leader to schedule a clinical and technical planning meeting to start the process of getting your hospital connected and trained.
