Pre-register for your hospital stay

Before your hospital stay, please complete a pre-registration form.

You can submit the form in one of three ways:

Fill out and submit the form online - you'll be directed to a secure form on the HealthEast web site
Print out the form (pdf) and mail it back to us
Send in the form you get from your clinic

It's important to consult your insurance company to determine your benefits and find out if you need pre-authorization.
