What the graph is saying

For this measure, lower percentages are better. The graph above shows that HealthEast hospitals had lower readmission rates than the MIDAS average for hospitals in the Midwest.

Why is this measure important?

This measure reflects the attempts by healthcare to reduce complications that are resource-intensive and inconvenient for the patient. Congestive heart failure is the most common indication for hospitalization among older adults. Interventions to reinforce patient education, ensure compliance with diet and medication, and identify recurrent symptoms amenable to treatment on an outpatient basis may be among the factors that can reduce readmission rates.

High rehospitalization rates drive growing costs and also indicate that current management approaches to heart failure are less than optimal.
