Outpatient Nutrition Referral form
What services are offered?
Registered dietitians provide medical nutrition therapy for a variety of diagnoses including hyperlipidemia, hypertension, obesity, malnutrition, celiac disease, and diabetes.
The initial nutrition session is one hour in length with half hour follow-up sessions. At the end of the session you will have a better understanding of how to change nutrition habits and food choices to meet your goals. The dietitian sends a written summary of the education visit and an outline of your dietary goals to your referring physician.
Adults (over the age of 18) are seen at all 3 locations. Children (age 6 and older) are also seen at Woodwinds Hospital.
Who provides these services?
Registered dietitians who are licensed by the State of Minnesota provide these services.
How to get a physician referral
Ask your physician to complete a HealthEast Outpatient Nutrition Referral form which includes your diagnosis, and fax it to HealthEast Scheduling Services. The fax number is on the form: 651-326-8516.
For a non-HealthEast physician, an order typed on letterhead paper or on a prescription pad will also work.
Will insurance cover it?
It is your responsibility to find out if your insurance company will pay for the dietitian visit. You will need to tell them your referring diagnosis and refer to the registered dietician visit as Medical Nutrition Therapy when inquiring.
Note: Medicare covers a registered dietitian visit only if the referring diagnosis is Diabetes or Kidney Disease.
