Pathways to Better Health
HealthEast offers the course, Pathways to Better Health, which is aimed at helping people with ongoing health conditions (such as diabetes) and their caregivers learn to manage their condition as well as feel and act healthier. A well-managed condition may help prevent long term problems.
This six-session program is designed to give the skills and motivation to:
- Become more active
- Eat better or stick to a diet
- Control fatigue or pain
- Cope with the emotional impact of cancer
- Communicate better with health care providers and family
