Nursing visit services

Our case managers have a broad range of nursing experience, and provide assessments, teaching and skilled nursing procedures. The case manager will work with your doctor, other team members, and caregivers to develop a plan of care. They will also evaluate your progress towards your goals and communicate these results with your entire team.

  • Home laboratory tests
    We can have laboratory specimens drawn or collected at home by a nurse and delivered to a certified laboratory for testing. Results are communicated to your doctor.

  • Home health aide (HHA)
    Our home health aides provide hands-on personal care. This may include bathing, assistance with walking and transferring, minor dressing changes and skin care.

Rehabilitation

HealthEast Home Care provides extensive rehabilitation services, including physical therapy, occupational therapy and speech therapy. Our goal is to restore or to increase your safety and independence.

Our home care rehabilitation team also includes social workers who help individuals and families manage the effects of illness by listening and working to address problems.

We commonly assist in:

  • Planning and coordinating personal care, homemaking, home-delivered meal and transportation
  • Reviewing options for assisted living and nursing home care
  • Advance directives
  • Social isolation and loneliness

Home infusion therapy

Many intravenous (IV) medications can now be safely administered in the home. Our specially trained nurses and clinical pharmacists work with your doctor to provide effective infusion therapies.

Private duty

Care can range from three hours to 24 hours a day, seven days a week. Private duty services are provided by home health aides.

Maternal/child health program

High risk pregnancies
Many women with high risk pregnancies can be monitored by a home care nurse in the comfort of their home to help prevent complications and early deliveries.

Services for new parents and babies
We have developed a program that provides individualized home care services to new parents and their babies. With today's shorter maternity stays, it is helpful and reassuring to have the support of a trained professional during the first days at home.

Palliative care

HealthEast Palliative Care provides home health care to those with advanced chronic illnesses, such as cancer, heart, lung, liver or neurological disease. We provide aggressive symptom management and support the body, mind and spirit.

HealthEast Health Alert

Health Alert is an in-home emergency response system that allows customers to live independently in their home with direct access to emergency assistance. We are always there, 24 hours a day, seven days a week, at the push of a button. For more information, call 651-232-3560.
