How proceeds will be used
Every day, patients are faced with navigating a health care delivery system that may be confusing and overwhelming. Health concerns for oneself or loved ones, combined with financial uncertainty, can lead to stress that is proven to block compliance to doctor’s orders.
Patient-centered care, such as HealthEast Clinics’ Medical Home, means treating patients as partners, involving them in planning their health care and encouraging them to take responsibility for their own health. This is an important aspect of quality, fiscally responsible care.
Funding from Festival of Trees will allow Clinics to:
- Advance patient-centered care in HealthEast Clinics
- Create patient education literatures and kiosks
- Establish Patient and Family Councils to partner with us as we develop new ways to structure care in the midst of health reform
- Secure employee development and education opportunities
