Community Health Needs Assessment
Responding to the health needs of the community, especially the most vulnerable among us, is core to achieving our HealthEast vision of optimal health and well-being. To do so, we must have an understanding of these needs and develop a deliberate approach to addressing the needs. An assessment is conducted every three years to help determine which programs and activities contribute to the health and vitality of our neighbors.
HealthEast conducted Community Health Needs Assessments (CHNA) in 2012 and 2015.

Community health improvement initiatives
The central goal of community health improvement is to produce measurable improvements in health status through strategic, evidenced-based programs. We will do this by partnering with community organizations.

- Health education
HealthEast offers a variety of classes and support groups for patients and the community.
- Health screenings
Periodically, HealthEast will host community health screenings, vein screenings, etc.
- Speakers bureau
As a not-for-profit organization, HealthEast has established criteria to ensure the best and most appropriate use of limited funds for sponsorships. Decisions are made by a committee on a monthly basis. Filling out the online sponsorship request form is required.
Community Relations refers to the establishment and maintenance of mutually beneficial relationships with people and organizations in the communities in which we operate. A comprehensive, ongoing community relations program gives this organization visibility as a good community citizen, supporting programs that improve the quality of life in our community, including social determinants of health.
