Community Health Needs Assessment

Responding to the health needs of the community, especially the most vulnerable among us, is core to achieving our HealthEast vision of optimal health and well-being. To do so, we must have an understanding of these needs and develop a deliberate approach to addressing the needs. An assessment is conducted every three years to help determine which programs and activities contribute to the health and vitality of our neighbors.

HealthEast conducted Community Health Needs Assessments (CHNA) in 2012 and 2015.

HealthEast’s fiscal year 2016-18 Community Health Implementation Plan describes our plan for improving community health, and how we will measure our impact. View the 2016-18 implementation plan.
