Internal Medicine Team-Based Care

The HealthEast Woodbury Clinic is implementing a new team-based model of care for some internal medicine patients.

Who is team-based care for?

Team-based care is for patients who see Dr. Stephan Burgeson or an internal medicine nurse practitioner at the Woodbury Clinic. Rather than working with one doctor or nurse practitioner, you will be working with Dr. Burgeson and a team of three nurse practitioners, all partnering closely with each other.

We believe in providing options to our patients and recognize that this new model may not fit for every patient's needs. Therefore, Dr. Tina Johnson will continue to deliver a traditional style of care and not move to a team-based model at this time.

How will it work for both healthy and sick patients?

Team-based models of care have been studied extensively and been proven to increase medical outcomes and patient satisfaction.

For patients who are healthy
Most of our patients are healthy, requiring less oversight and management than patients who are more sick. But this doesn't mean they should get less attention. By creating a team of nurse practitioners, we can work with patients to stay as healthy as possible and focus on prevention and early detection of illness.

Our nurse practitioners have a wealth of knowledge and skills. They will help you achieve your health-related goals. They work hand-in-hand with you to help you make wellness and lifestyle changes that can impact your long-term health.

For patients with more complex conditions
If you have a complex or difficult condition, a team-based model helps ensure that you receive the high level of care oversight and coordination that your health requires. With a team-based approach, we will be better able to ensure that your conditions are tightly monitored and managed.

What will happen at my next appointment?

When you call to for an appointment, we will ask if you want to see a particular nurse practitioner or if you would like the first available appointment. No matter which option you choose you will:

  1. Meet first with a nurse practitioner to review your history and current situation.
  2. Dr. Burgeson will then join the visit to discuss your concerns and ask any further questions. Together, you'll decide on a plan for care.
  3. Your nurse practitioner will then prescribe any medications that may be necessary, provide more information and answer any additional questions.

How can I help?

We know this is a significant change for you. It is also a big change for our clinic staff. Please be patient as we begin using this new model of care.
