Through HealthEast Medical Care for Seniors, doctors and geriatric nurse practitioners make "house calls" to nursing homes, assisted living facilities and transitional care units.
Caring for patients and residents where they live, our services eliminate the need to travel to a primary care clinic. Our providers are trained to meet the unique physical and social needs of seniors.
Our services
Our team visits patients and residents on a regular basis to provide basic preventive care services to keep them healthy. They also provide diagnosis, monitoring and ongoing treatment for illnesses and injuries. Our team is also available to see patients outside of regular appointments, when needed.
Services include:
- Ongoing primary care
- Review of medical history and general physical exams
- Assessments to prepare for surgery
- Short-term care after hospital discharge (transitional sub-acute care)
- Hospice Care
- Access to specialty providers and facilities when a higher level of care is needed
- Medical directorship and consultation
Where we see patients
Nursing homes
For the convenience of the residents, they are seen by our team in their rooms.
Assisted living facilities
Our team visits residents at satellite clinics in assisted living facilities, allowing residents to stay in the building for their health care needs.
Transitional care units
Transitional care is for patients who have been released from the hospital and are recovering in a transitional care facility before returning home. Our team acts as a bridge between the hospital and the patient's primary physician. Our doctors (also called "extensivists") are specialists in this type of care, which has been shown to help shorten the length of stay for patients.
Service area
Providers with HealthEast Medical Care for Seniors visit more than 40 nursing and transitional care centers, as well as a growing number of assisted living facilities throughout the East Metro area.

When additional services are needed, we are dedicated to making the journey through the health care system as smooth as possible. Through Medical Care for Seniors, patients and residents have access to a full range of facilities and services, including:

Emergencies
In case of emergencies, physicians and nurse practitioners are accessible by phone 24 hours a day/7 days a week.
