At HealthEast Clinics, we partner with you to help you achieve optimum health. You can use the health asssessments below to assess how you are managing your condition. When you share the information with your provider, it also helps guide him or her to identify areas of opportunity to improve your overall health.
HealthEast Clinics offer a full spectrum of care and we look forward to serving you. If any of these assessments indicate an opportunity for action, then we invite you to make an appointment with your clinician to discuss your care.
Asthma
Who's in control … you or your asthma? The Asthma Control Test™ assesses your level of asthma control. At the link below, please locate the "Toolbox" in the center of the page and select "Asthma Assessment." Then select "For Adults" or "For Children" and complete the assessment tool. When you are done, be sure to review your results with your provider.
