There are 3 easy ways to refer a patient:
When referring patients
Referrals are subject to an on-site evaluation conducted by a Bethesda Referral Specialist. Our liaisons work closely with families and discharge planners to ensure that the admission process is smooth. In addition, they can aid in answering financial and insurance questions.
Please have the following information available so that your request can be processed efficiently:
- Patient's personal information (name, address, phone, other contact information)
- Patient's medical information (condition, cause, medical course)
- Patient's financial information (insurance, etc.)
Bethesda Hospital admission criteria
Bethesda Hospital works with acute care hospitals, group homes and skilled nursing facilities throughout the Midwest and treats patients with:
- Spinal cord injury
- Lung diseases
- Traumatic and acquired brain injury
- Stroke
- Neurological disorders
- Non-healing wounds
- Concurrent medical and behavioral problems.
All referrals have to be approved by Bethesda Hospital prior to admission. All admission decisions are made on a case-by-case basis and are based on:
- Diagnosis
- Medical condition
- Rehabilitation goals
- Funding requirements
- Anticipated discharge disposition. Bethesda Hospital's programs are designed for patients, 16 years of age or older.
