Qualifying Condition(s): Multiple conditions including respiratory failure, tracheotomy, surgical site wound, perforated stomach, malnourished, delirious, de-conditioned, in constant pain, depressed
Treatment: Integrated, multispecialty approach including continuous infusion of pain medications, proper nourishment, ventilator/trach weaning with daily breathing training and education, wound care management with wound VAC, physical therapy, TCU for additional recuperation
Length of Stay: 2 months
Outcome: Alert and aware with minimal pain and anxiety, overcame respiratory distress, improved eating, progressed healing of surgical wound, ability to dress and groom herself, discharged only needing assistance of a walker
Qualifying Condition(s): Diagnosed with multiple myeloman, post-operative complications after bone marrow transplant, need for frequent evaluations
Treatment: Coordinated, interdisciplinary, personalized plan of care, intensive customized medical and therapeutic treatments, puzzles and mind teasers, family care conference
Length of Stay: 1 month
Outcome: Discharged to home with his family
Qualifying Condition(s): Medically induced coma following pneumonia, acute respiratory distress syndrome (ARDS), staph infection, ventilator and trach, unable to walk, talk or write, medically complex needs
Treatment: Personalized plan, respiratory care, strength building, complex medical care, wound care (VAC), family care conference, physical therapy, speach therapy, occupational therapy, recreational therapy, pet therapy, TCA IV, HealthEast Home Care, future surgery for fistula
Length of Stay: 2.5 months
Outcome: Regained her ability to walk, talk, write, use the computer and handle everyday activities, discharged to home
