Our vision for HealthEast is “Optimal health and well-being for our patients, our communities and ourselves.” As we strive toward our vision, HealthEast will become a vital partner, woven into the fabric of the community creating a healthy, vibrant and productive East Metro area. We will serve our community in leadership, partnership, strength and energy, bringing our expertise in medical care, population health, and management of chronic disease to our community so we create a great place to live.
Our unique role in the community will expand beyond medical care to having expertise and developing partnerships focused on what makes good health. Our end goal is for our community to get better and better, leading to healthier, productive living, creating economic change that allows the fullness of the community to come alive, bringing in arts, education and other programs that make a community vibrant.
Community Health Needs Assessment
To understand our community better, HealthEast conducted a Community Health Needs Assessment in 2012. Learn more about our Community Health Needs Assessment and Implementation Plan or read this Community Health Needs Assessment presentation.
Community health improvement
The central goal of Community Health Improvement is to produce measurable improvements in health status through strategic, evidenced-based programs. We will do this by partnering with community organizations.
Community health promotion and awareness
Community relations
Community Relations refers to the establishment and maintenance of mutually beneficial relationships with people and organizations in the communities in which we operate. A comprehensive, ongoing community relations program will give the organization visibility as a good community citizen when they support programs that improve the quality of life in their community, including social determinants of health.
